The Supply Request Form is intended for the use of suburban MLS Member Libraries, and is used to request delivery routing lables and Your Public Library Card brochures. One of the easiest ways to promote Interlibrary Loan and Reciprocal Borrowing at your library is to have a supply of these brochures available for your patrons.
