ILL Request Form (loans)

The International Interlibrary Loan Request Form is intended for the use of the MLS Member Libraries only. International ILL will be performed on a cost recovery basis. Please be sure to indicate the maximum amount your patron is willing to pay on the form.

The Interlibrary Loan Verification Form is intended for the use of MLS Member Libraries only. If you are experiencing any difficulty in verifying a book, CD, DVD, thesis, dissertation, or foreign publication- just give us a call or use the verification form on the website.

MLS Member Libraries should exhaust SWAN, I-Share (formerly Illinet Online), and WorldCat Resource Sharing resources before using either of these forms to direct a request to MLS Resource Sharing.

Photocopy Request Form (copies)

The International Photocopy Request Form is intended for the use of MLS Member Libraries only. International photocopies will be performed on a cost rebovery basis. Please be sure to indicate the maximum amount your patron is willing to pay on the form.

The Photocopy Verification Form is intended for the use of MLS Member Libraries only. If you are experiencing any difficulty in verifying a photocopy request or a journal title - just give us a call or use the verification form on the website.

Libraries should attempt to fill requests for journal articles by using the full text resources available in FirstSearch. Before submitting a verification request to MLS Resource Sharing, members should exhaust the MLS Union List of Serials and FirstSearch.

Supply Request Form

The Supply Request Form is intended for the use of suburban MLS Member Libraries, and is used to request book bands and Your Public Library Card brochures. One of the easiest ways to promote Interlibrary Loan and Reciprocal Borrowing at your library is to have a supply of these brochures available for your patrons.

Resource Sharing Site Visit Request Form

The Resource Sharing Site Visit Request Form is intended for the use of suburban MLS Member Libraries to request a site visit from the MLS Resource Sharing staff. MLS Resource Sharing staff is available to visit your library and train staff on numerous topics - including Interlibrary Loan and Photocopy resources, locating full text articles, ILL workflow, updating the MLS Union List of Serials, and Reciprocal Borrowing.
